Fill out our brief Discovery Call form (takes less than 5 minutes to complete).
Our medical team will review your information to decide the right package or sequence of packages would be best for you.
A licensed medical professional will review your intake form for any contraindications.
Once you’ve been approved, your testing will be processed.

TESTING KITS DELIEVERED TO YOUR DOOR!

WEEKS 1-2

HEALTH INTAKE & ONBOARDING

After joining, you'll have access to our portal where you can start to fill out your forms, intakes and questionnaires.
You will have IMMEDIATE access to our On-boarding course, where you can learn to maximize your experience.
Schedule an On-boarding call with our team at your earliest convenience.

WEEKS 3-4

INITIAL VISIT & HEALTH PLAN

Come meet our medical team in person to conduct an in person exam, review all your labs, testing, forms and treatments.
Meet your board-certified physician. Together with your health coach, you'll review your genomics, labs, health goals, and Personal Health Report.
We will formulate your personalized treatment strategy with you as a partner.

WEEKS 5-12

PROGRESS & OPTIMIZATION

With your personalized health plan in motion, you will track progress and make adjustments to optimize your health
Continue to make lifestyle changes in increments with the continuous support of your coach and dedicated care team.
More physician touch points at your pace. There are no time restrictions. Go at your pace.
Our accredited health coaches offer accountability and support in implementing the clear and actionable interventions within your care plan.
